4. Two (2) identical Passport Pictures
Must be 2x2’s with light background. (No lettering or photos on
clothing, no eyeglasses)
5. State issued ID: Driver License or ID Card. (may use Military ID)

EXPEDITE SERVICE
You may expedite your application to receive your passport between 5 and 7 weeks (regular 6-8 weeks). The Fee is an additional $60.00 for a total of $190.00 (Adult) or $160.00 (Children) Made Payable To U.S. Department of State, plus $35.00 in cash (The Passport Card can not be Expedited.)
NOTE:
1. ALL CHILDREN UNDER 16 YEARS OF AGE MUST HAVE BOTH
PARENTS CONSENT FOR A PASSPORT.
2. ALL PERSONS, ADULT OR CHILDREN, MUST BE PRESENT AT
TIME OF APPLYING FOR A PASSPORT.
3. Passports are good for (10) years. If you are under age 16, Passport is good
for (5) years.
